Services we offer When Liaising with NASC

 

• Referral action appropriately and efficiently with no unnecessary delays.
   

• Involvement  in the Needs Assessment, ensuring all appropriate information is captured.
   

• Liaison directly with appropriate Disability Support Services (DSS) in the community to minimise delays.
   

• Liaison directly with NASC to determine funding of appropriate disability support services is allocated.
   

• Assist with Self Assessments.
   

• Support Clients and Families during changes to Transformation.
   

• Follow up further Reviews.
